Welcome to Yucaipa Valley Pediatric Living, Inc. a licensed Congregate Living Health Facility (CLHF) dedicated to providing exceptional, around-the-clock care for the medically fragile pediatric population in a nurturing home environment. ​

A Congregate Living Health Facility (CLHF) is a State-licensed residential home in California that provides inpatient care and is designed to serve residents whose primary need is for skilled nursing care on a recurring, intermittent, extended, or continuous basis. CLHFs offer a level of care that is less intensive than a general acute care hospital, but more specialized than that provided in a traditional skilled nursing facility.

Each CLHF provides medical supervision, 24-hour skilled nursing and supportive care, pharmacy, dietary, social, and recreational services, and at least one of the following types of specialized care:

  • Services for individuals who are mentally alert but physically disabled, including those who may be ventilator dependent.

  • Services for individuals with a terminal or life-threatening illness, as certified by their physician.

  • Services for individuals who are catastrophically and severely disabled, often due to trauma or non-degenerative neurological illness, and who benefit from ongoing rehabilitative therapies such as speech, occupational, and physical therapy.

At Yucaipa Valley Pediatric Living, Inc. our home has been carefully renovated and designed to provide a safe, comfortable, and healing environment that feels like home — not a hospital. Our facility features six bedrooms, including two private rooms for individual patients and two shared rooms designed for two children each. Every detail, from accessibility features to cheerful décor, was created with the comfort, dignity, and emotional well-being of our young residents in mind.​ Nestled in the beautiful and peaceful upper Yucaipa community, our home offers a quiet, natural setting where children can heal, grow, and thrive.

 

Our team of dedicated nurses, caregivers, and therapists provides 24-hour skilled nursing care while ensuring each child feels loved, valued, and at home. We believe that healing extends beyond medical care — it’s built on connection, compassion, and the joy of being part of a family.

We deeply value family interaction and collaboration as essential components of every child’s recovery and well-being. Families are encouraged to participate in care, share special moments, and remain closely involved in their child’s journey toward health and independence. 

Our Story

Yucaipa Valley Pediatric Living, Inc.  began with a simple but powerful calling — to provide medically fragile children with not just exceptional medical care, but also the warmth, comfort, and love of a home.

As foster parents to medically fragile children, Hanah and Thomas Malcolm saw firsthand how transformative a nurturing, family-centered environment can be. As a Registered Nurse, Owner, and Administrator, Hanah has cared for children with complex medical needs — including those with tracheostomies, feeding tubes, and ventilator dependence — and has witnessed the incredible difference that a loving home environment makes in their health and happiness.

Through their fostering journey, Hanah and Thomas realized there were so many other children in their community who deserved that same opportunity — to receive the care they need in a place that feels like home. That vision became the heart of Yucaipa Valley Pediatric Living, Inc.

What started as a dream built on compassion has grown into a reality — a warm, specialized home where children can heal, grow, and be surrounded by love. Together, Hanah and Thomas are honored to continue this journey and serve even more children and families with open hearts and caring hands.
